1. Ocular Lyme Diseases This retrospective case series synthesizes 27 published reports of ocular Lyme disease between 1988 and 2025, detailing 38 patients with diverse eye manifestations. Uveitis accounted for 45% of cases, followed by optic neuritis and ocular motor cranial nerve palsies; A total of 5 cases had microbiologic confirmation of Borrelia burgdorferi by PCR, culture, or direct visualization, underscoring both the clinical heterogeneity and diagnostic challenges of ocular Lyme disease and the need for targeted microbiologic testing when eye findings are otherwise unexplained. Comment: Lyme disease remains the most frequently reported tick-borne infection in the United States, with more than 89,000 cases officially reported to the CDC in 2023, while modeling suggests that the true annual burden may approach 476,000 cases, meaning that rare clinical courses of the infection may be overlooked, and this manuscript specifically highlights ocular disease manifestations.
